Unique Partnership with New York's Largest Multicultural Physician-Led Network Integrates Multiple Risk-Management Programs into a Single Platform

SOMOS Community Care, The Garage and Optimus join forces to radically simplify data and unify care management for five different entities

ORLANDO, Fla.--(BUSINESS WIRE)--The Garage, an advanced population health management technology company providing patient-centered digital solutions to stakeholders across the health care industry, joins SOMOS Community Care and Optimus Solutions in announcing their groundbreaking partnership. Together, the organizations will seamlessly increase and optimize care quality and operations across multiple high-risk management programs in New York City through one unified platform.

SOMOS, New York's largest physician-led network serving Latino and Asian-American populations, and its in-house IT consulting firm, Optimus, will leverage The Garage's population health platform, Bridge, to integrate all providers, patients and population health solutions into one knowledge engine for advanced data analytics and centralized care management. The unique structure of the collaboration allows SOMOS to house its data with Optimus, which the Garage will use to become SOMOS's population health platform and data analysis center. The partnership will unify SOMOS's nearly 3,000 physicians and impact a total population of over 2 million patients, primarily made up of Medicaid.
